The short answer: Yes. In controlled trials, a single 480 mg dose of caffeine (about five cups of coffee) triggered a panic attack in about half of people with panic disorder. Only about 2% of healthy adults reacted the same way. The FDA sets 400 mg a day as the ceiling for most adults, but people prone to panic often need to stay well below it. A single 150 mg cup did not raise anxiety in the one trial that tested it.
Can caffeine really trigger a panic attack?
Researchers have used caffeine to provoke panic attacks in the lab for 40 years. A 2022 systematic review and meta-analysis in General Hospital Psychiatry pooled ten placebo-controlled caffeine challenge studies. Across the nine studies that tracked panic attacks, 237 people with panic disorder received caffeine. A total of 51.1% had a panic attack after caffeine. None had one after placebo.
The contrast with healthy people is large. In the six studies with a comparison group, 53.9% of patients panicked after caffeine versus 1.7% of healthy controls. Caffeine also raised subjective anxiety more in patients than in controls. The doses ranged from 400 mg to 750 mg, with most studies using 480 mg. The authors call that "roughly equivalent to 5 cups of coffee."
One of those studies shows the pattern in detail. A 2007 trial in Comprehensive Psychiatry gave 480 mg of caffeine in coffee form to four groups, one week apart from a caffeine-free placebo. A panic attack followed in 58.6% of people with panic disorder, 44.4% of people with depression plus panic attacks, 12.0% of people with depression alone, and 7.1% of healthy volunteers. No one panicked after the placebo.
The oldest study on the list is the one most people cite. In 1985, Charney and colleagues gave 10 mg of caffeine per kilogram of body weight to 21 panic patients and 17 healthy subjects. Patients had much larger jumps in anxiety, fear, nausea, palpitations, and tremor. Seventy-one percent of the patients said the caffeine felt like their real panic attacks.
Why does caffeine feel like panic?
Caffeine blocks adenosine receptors in the brain. Adenosine is a calming signal that slows nerve firing. When caffeine sits in those receptors, the brake comes off. The 2022 review notes that these receptors help regulate arousal and anxiety. Charney's 1985 paper suggested that some panic patients may have differences in this adenosine system, which would explain their outsized reaction.
The second reason is the body. The FDA lists increased heart rate, palpitations, jitters, anxiety, nausea, and insomnia as signs of too much caffeine. Compare that with the NIMH list of panic attack symptoms: pounding heart, sweating, trembling, difficulty breathing, dizziness, and stomach pain or nausea. The lists overlap almost completely.

How much caffeine is too much?
For the general adult population, the FDA cites 400 mg a day as the amount most healthy adults can consume without harmful effects. It describes that as about two to three 12-ounce cups of coffee. MedlinePlus uses the same 400 mg figure. Both agencies say individual sensitivity varies. MedlinePlus lists people with anxiety among the groups who should limit caffeine or avoid it.
For people who are prone to panic, the evidence points lower. A 2024 meta-analysis in Frontiers in Psychology looked at 546 healthy people with no psychiatric diagnosis. Caffeine raised anxiety scores with a moderate effect below 400 mg and a very large effect at 400 mg or more. If caffeine can do that to people without an anxiety disorder, a panic-prone brain has less margin.
Three more numbers set the edges:
- 200 mg a day in pregnancy. ACOG says less than 200 mg a day does not cause miscarriage or preterm birth. That is one 12-ounce cup of coffee.
- About 1,200 mg at once. The FDA says toxic effects such as seizures can occur with rapid intake near this level. That is less than half a teaspoon of pure caffeine powder.
- Timing. MedlinePlus says caffeine peaks in the blood within one hour and the effects last four to six hours. A 4 p.m. coffee is still active at bedtime.
How much caffeine is in your cup?
Most people undercount. The FDA gives these ranges per 12 fluid ounces:
- Regular brewed coffee: 113 to 247 mg
- Energy drink: 41 to 246 mg
- Black tea: 71 mg
- Green tea: 37 mg
- Caffeinated soft drink: 23 to 83 mg
- Decaf coffee: 2 to 15 mg per 8 ounces
A strong 12-ounce brewed coffee can hold 247 mg. Two of those in a morning is close to the 480 mg challenge dose that panicked half the patients in the trials. A 16-ounce or 20-ounce shop coffee is not one serving, so scale the numbers up. Energy drinks vary most, so read the label and note the can size. Caffeine also hides in chocolate, some pain relievers, and pre-workout powders. Count all of it.
Does a normal cup of coffee cause panic?
The panic studies used large doses, so they do not settle what one cup does. A 2025 randomized crossover trial in the Journal of Psychopharmacology gave 150 mg of caffeine or placebo to 29 people with panic disorder and 53 healthy controls. All were low habitual users who abstained for 36 hours before each session.
The result surprised the authors. The dose did not raise subjective anxiety in either group. Only one panic attack occurred, in a patient during an emotional task. Caffeine did raise skin conductance (a marker of arousal) and avoidance behavior in both groups. The authors concluded that advice to avoid caffeine in panic disorder "should be based on higher doses and ideally on individual assessments."
So a single small cup is unlikely to launch a panic attack by itself. Several cups, or one very large one, can. Sleep sits in between. Should you quit caffeine if you have panic attacks?
Not always, but you should test it. Charney's 1985 paper ended with a plain suggestion: "Patients with anxiety disorders may benefit by avoiding caffeine-containing foods and beverages." The 2022 review says clinicians should tell panic disorder patients about caffeine's effects, with the caveat that little is known about doses under 480 mg. A practical approach:
- Track for a week. Write down every caffeinated item, the amount, the time, and any anxiety or panic that followed.
- Cut to under 200 mg a day, before noon. Taper over one to two weeks. MedlinePlus lists headache, drowsiness, irritability, and nausea as withdrawal symptoms that fade in a couple of days.
- Judge the result after two to four weeks. Fewer attacks or calmer mornings point to caffeine as a trigger.
- Tell your clinician. The DSM-5 lists caffeine-induced anxiety disorder and caffeine intoxication as formal diagnoses, as the NCBI Bookshelf notes. Caffeine can mimic panic disorder or worsen it. Your clinician needs your intake to sort out which.
Quitting caffeine treats one trigger. It does not treat panic disorder. NIMH lists cognitive behavioral therapy, SSRIs, SNRIs, and in some cases beta-blockers or benzodiazepines as the treatments with evidence. The bottom line
Caffeine can trigger panic attacks, and the effect is strongest in people who already have panic disorder. In placebo-controlled trials, 480 mg (about five cups of coffee) produced a panic attack in about half of patients and in under 2% of healthy adults. The FDA's 400 mg daily ceiling is a limit for the general public, not a target for someone who panics. A 150 mg cup did not raise anxiety in the one trial that tested it. Add up your real daily total, keep it low and early, and track what happens. If panic attacks continue without caffeine, treatment for panic disorder works.
