The cuff inflates, the number comes up higher than you expected, and your pulse jumps in response. On the second reading it is higher still. By the third you are convinced something is wrong.

This is one of the most common questions in anxiety care, and the honest answer has two halves that get confused with each other constantly.

The short answer

Anxiety definitely raises blood pressure in the moment. That part is not controversial. Sympathetic activation increases heart rate and constricts blood vessels, and the reading goes up.

Whether anxiety causes sustained hypertension is the harder question, and the evidence leans yes. A 2015 systematic review and meta-analysis pooling eight prospective studies, 80,146 participants, and 2,394 new hypertension cases found an adjusted hazard ratio of 1.55 for developing hypertension among people with anxiety.

So both things are true, and they need different responses. Short-term spikes need context. A 1.55 hazard ratio needs long-term management.

What anxiety does to blood pressure in the moment

The mechanism is the acute stress response, and it is doing exactly what it evolved to do.

Adrenaline and noradrenaline are released. Heart rate rises and each beat ejects more blood. Peripheral vessels constrict, raising resistance. Both changes push the number up, sometimes sharply, within seconds.

During a panic attack the effect can be dramatic. Readings that would prompt urgent evaluation if sustained can appear and then resolve entirely as the episode passes. The key feature is that transience. A blood pressure that is 165/100 during panic and 118/76 an hour later is a story about the panic, not about your arteries.

Anxiety also produces symptoms people attribute to high blood pressure: headache, chest tightness, palpitations, and dizziness. Uncomplicated hypertension usually causes no symptoms at all, which is why it is called a silent condition. Does anxiety cause lasting hypertension?

Probably, though the evidence is observational rather than experimental.

The 2015 meta-analysis is the most useful single number: a pooled adjusted hazard ratio of 1.55, with a 95% confidence interval of 1.24 to 1.94. Meta-regression found that location, anxiety diagnostic criteria, age, sex, sample size, publication year, study quality, and follow-up length did not explain the heterogeneity, which is a point in favor of the association being real.

Earlier longitudinal work found similar direction. In the NHANES I Epidemiologic Follow-up Study, high anxiety carried a relative risk of 1.82 for incident hypertension among white participants aged 45 to 64, and 2.74 among Black participants aged 25 to 64.

Two caveats keep this honest. These are observational cohorts, so anxiety may travel with other risk factors rather than cause the outcome. And several plausible pathways are indirect: anxiety disrupts sleep, reduces exercise, increases smoking and alcohol use, and worsens medication adherence. Each of those independently raises blood pressure.

White coat hypertension: the most common overlap

This is where anxiety and blood pressure intersect most often in everyday life.

White coat hypertension means readings that are elevated in a clinical setting but normal outside it. Prevalence figures vary a great deal depending on the threshold applied. In one analysis of 7,353 people with elevated office readings, 17.4% met criteria under the 2018 ESC/ESH guidelines and 9.4% under the 2017 ACC/AHA guidelines. Broader estimates put the global range at 15% to 30%.

It is not harmless and it is not the same as normal blood pressure, since people with it carry somewhat higher long-term cardiovascular risk than people who are normotensive everywhere. But it is a different problem from sustained hypertension and it should not be treated with the same reflex.

The only way to identify it is measurement outside the office.

How to tell which one you have

Home or ambulatory monitoring, done properly. The protocol matters more than the device:

  1. Sit quietly for 5 minutes first, back supported, feet flat, arm at heart level.
  2. No caffeine, exercise, or smoking in the preceding 30 minutes. Empty your bladder.
  3. Take two readings a minute apart and record both.
  4. Do this twice a day, morning and evening, for about a week.
  5. Discard the first day and average the rest.

Use a validated upper-arm cuff. Wrist devices are considerably less reliable.

One behavioural rule matters as much as the technique: do not re-check because you did not like the number. Repeat checking within minutes reliably produces higher readings and reinforces the anxiety that produced them. Record what the schedule gives you and interpret the average, not any single value.

When anxiety medication itself raises blood pressure

Worth knowing, because it is a real and specific effect rather than a vague warning.

Venlafaxine, an SNRI widely used for generalized anxiety disorder, raises blood pressure in a dose-dependent way. Its FDA labeling reports a mean increase in supine diastolic blood pressure of 7.2 mm Hg at 375 mg per day at week 6, with essentially no change at 75 or 225 mg per day. Across clinical studies, 1.4% of patients on extended-release venlafaxine had a sustained diastolic rise of 15 mm Hg or more to a level of 105 mm Hg or above, versus 0.9% on placebo.

The practical reading: this is mostly a high-dose issue, and it is a reason for blood pressure monitoring when doses are increased rather than a reason to avoid the drug. SSRIs do not share this effect to a meaningful degree.

Two other common contributors are worth checking before concluding anything: stimulants prescribed for ADHD, and decongestants containing pseudoephedrine. When the arrow points the other way

Sometimes the anxiety is the symptom rather than the cause, and this is worth ruling out if the presentation is unusual.

Hyperthyroidism produces anxiety, palpitations, tremor, and elevated blood pressure together. Pheochromocytoma, a rare adrenal tumour, classically causes episodic surges of blood pressure with headache, sweating, and a sense of doom that closely resembles panic. Obstructive sleep apnea causes both nocturnal hypertension and daytime anxiety.

Features that argue for investigation rather than reassurance: blood pressure surges that arrive out of nowhere without a psychological trigger, drenching sweats during episodes, unexplained weight loss, or a first panic-like episode after age 45.

What actually helps

The useful part of this overlap is that several interventions work on both problems at once:

  • Aerobic exercise. Reduces both anxiety symptoms and resting blood pressure, with effect sizes that are meaningful for each.
  • Sleep. Short and fragmented sleep raises blood pressure and worsens anxiety. Both improve together when sleep does. See our guide to anxiety-driven insomnia.
  • Alcohol reduction. Alcohol raises blood pressure directly and worsens rebound anxiety the following day.
  • Caffeine timing. Caffeine acutely raises blood pressure and lowers the threshold for panic in sensitive people.
  • Treating the anxiety itself. CBT and appropriate medication reduce the frequency of spike episodes, which is the part of the problem that anxiety is genuinely causing.

When to seek urgent care

A very high reading with symptoms is not something to talk yourself out of. Seek immediate care for a reading at or above 180/120 accompanied by chest pain, shortness of breath, back pain, weakness or numbness, vision change, or difficulty speaking. That combination is a hypertensive emergency and needs evaluation regardless of how anxious you feel.

A reading at or above 180/120 without those symptoms still warrants prompt medical contact, but it is not the same emergency.
