The short answer: Hydroxyzine works faster. Its FDA label says effects usually show within 15 to 30 minutes, so it suits short-term, as-needed relief. Buspirone takes about 2 to 4 weeks to help and must be taken every day, so it fits ongoing generalized anxiety. Hydroxyzine brings more drowsiness and heart-rhythm cautions, and older adults are usually told to avoid it.

How do buspirone and hydroxyzine work differently?

These two drugs share almost nothing except the goal of easing anxiety. They act on different brain systems, and that explains most of the gap in speed and side effects.

Hydroxyzine is a first-generation antihistamine. MedlinePlus says it blocks histamine and decreases activity in the brain. The FDA-approved label describes it as unrelated to benzodiazepines. It may act by quieting certain areas below the brain's cortex. It also has antihistamine, anti-nausea, and mild muscle-relaxing effects.

Buspirone is an anxiolytic in the azapirone class. Its label says the exact mechanism is unknown. It binds strongly to serotonin 5-HT1A receptors and moderately to dopamine D2 receptors. It has no meaningful effect on benzodiazepine or GABA receptors. The label adds that it lacks the strong sedation of typical anxiety drugs.

What is each one FDA-approved for?

The labels are worded differently, and the wording matters.

  • Hydroxyzine: "symptomatic relief of anxiety and tension associated with psychoneurosis and as an adjunct in organic disease states in which anxiety is manifested." It is also approved for itching from allergic skin conditions and as a sedative around surgery.
  • Buspirone: "the management of anxiety disorders or the short-term relief of the symptoms of anxiety." Its efficacy trials enrolled outpatients whose diagnosis roughly matched generalized anxiety disorder (GAD).

Both labels hedge on long-term use. Hydroxyzine's label says its effectiveness for more than 4 months "has not been assessed by systematic clinical studies." Buspirone's label says controlled trials have not shown effectiveness beyond 3 to 4 weeks. It notes, however, that 264 patients took it for 1 year without ill effect. Both labels tell doctors to reassess usefulness periodically.

Which one works faster?

Hydroxyzine, by a wide margin. The label states it is rapidly absorbed, with clinical effects usually noted within 15 to 30 minutes of an oral dose. That is why it is often prescribed for a tense afternoon or a hard evening.

Buspirone is absorbed quickly too, with peak blood levels 40 to 90 minutes after a dose. The calming effect lags far behind. A StatPearls review on NCBI Bookshelf says it "has little efficacy as an acute anxiolytic" because the clinical effect typically takes 2 to 4 weeks. Which one is meant for daily use?

Buspirone is built for a daily schedule. MedlinePlus describes twice-daily dosing, taken consistently either always with food or always without. The label explains why: food raised peak levels of unchanged buspirone by 116% in a small study. Doses are raised gradually, no more often than every 2 to 3 days.

Hydroxyzine's label lists dosing up to four times a day, and its fast onset also makes as-needed use possible. The evidence for steady long-term use is thin. A Cochrane review of hydroxyzine for GAD found only 5 usable trials with 884 participants. It rated the risk of bias as high. The authors concluded hydroxyzine cannot be recommended as a reliable first-line GAD treatment.

Buspirone has a larger evidence base. A Cochrane review of azapirones pooled 36 trials with 5,908 participants. Azapirones beat placebo, with a number needed to treat of 4.4. Most trials lasted only 4 to 9 weeks, and the authors called for longer studies. In the small head-to-head data, hydroxyzine and buspirone showed no clear efficacy difference (odds ratio 0.76, 95% CI 0.40 to 1.42).

How do the side effects and safety warnings compare?

Hydroxyzine carries the heavier warning list:

  • Drowsiness. The label warns against driving until you know your response. The Cochrane review found more sleepiness than with comparison drugs (odds ratio 1.74).
  • QT prolongation. The label reports post-marketing cases of QT prolongation and Torsade de Pointes. It is contraindicated with a known prolonged QT interval. Caution applies with heart disease, electrolyte problems, and QT-prolonging drugs such as citalopram, azithromycin, methadone, and ondansetron.
  • Anticholinergic effects. Dry mouth is the listed effect. Alcohol and other sedatives add to its impact.
  • Pregnancy. The label lists early pregnancy as a contraindication. It also says not to give it to nursing mothers.
  • Rare skin reaction. Acute generalized exanthematous pustulosis (AGEP) can occur. Stop the drug at the first sign of a rash.

Buspirone is less sedating, according to its label. It carries different warnings. It must not be combined with MAO inhibitors, including linezolid and IV methylene blue, because of serotonin syndrome and blood pressure risk. Restlessness shortly after starting has been reported in a small fraction of patients. Grapefruit juice in large amounts should be avoided.

Who does each one suit, and what about dependence?

Neither drug is a benzodiazepine. Buspirone's label states it is not a controlled substance. In human and animal studies it showed no abuse potential, tolerance, or physical dependence. One practical catch: it does not prevent benzodiazepine withdrawal, so those drugs need a gradual taper first. Hydroxyzine's label has no drug abuse and dependence section, but it does warn that it adds to the effects of alcohol and other sedatives.

In broad terms, the evidence points this way:

  • Buspirone fits people with ongoing, day-to-day GAD who can wait a few weeks. It also fits people who want to avoid sedation or dependence risk.
  • Hydroxyzine fits younger, healthy adults who need occasional fast relief. It also suits people whose anxiety comes with itching or poor sleep, when drowsiness is acceptable.

Age changes the picture. The 2023 American Geriatrics Society Beers Criteria list hydroxyzine among first-generation antihistamines to avoid in adults 65 and older (strong recommendation). The panel cites confusion, constipation, and higher risk of falls, delirium, and dementia from anticholinergic drugs. MedlinePlus adds that older adults usually should not take hydroxyzine.
